Date of death for *second* source case activated if first source case marked dead

description

On the short form, section 4, question 1, if "Dead" or "Alive and dead" is selected in the right most column for the 1st source case, the "Date of death" field becomes active for both the first and the second source case in the column. Likewise if "Alive" is selected for the first short case, the "Date of death" field is inactivated for both the first and the second source case, even if the second source case has been marked dead or alive and dead.

Desired behavior: The "Date of death" field for the second source case should be activated or inactivated based on whether the second source case is marked Alive, Dead, or Alive and Dead. The status of the first source case should not determine whether the "Date of death" field for the second source case is active.

Steps to replicate:
  1. Open an international database (French or English) and open or create a case using the short form.
  2. Scroll to section 4. In the table in question 1, choose "Dead" in the right-hand column for the first row. Observe that the "Date of death" field is now active in both the first and the second rows of the table.
  3. Now choose "Alive" in the first row, right-hand column. Observe that the "Date of death" field is now inactive in both the first and the second rows of the table.
  4. Go to the second row of the table and choose "Dead" in the right-hand column. Observe that this does not activate the "Date of death" field in the second row of the table.
Issue identified in 9.5.5, both French and English language settings.
