US version - Dates appearing in DD/MM/YY format

When the culture settings in the application are set to US, many dates are still being automatically formatted as DD/MM/YY. This is specifically happening in the following places:
  1. ALL dates in ALL linelists
  2. All dates on the daily and prior follow-ups screens
  3. All dates on the contact tracing printouts (Daily report and 21-day report available on the daily follow-up screen)
  4. All dates on the 3 case printouts under the "Print" menu button on the Case Management tab
Desired behavior: When the culture settings are set to English/US, all dates should automatically be formatted as MM/DD/YY.

Update 12/16/14: Some parts of this requirement no longer apply to the US settings of the app due to other app updates and so have been removed from the requirement.

I found a couple of places where the dates are still in DD/MM/YY format in 9.5.1:
  1. In the lab record linelist that appears when choosing "Show Lab Records" from the action wheel on a case on the Case Management tab, the dates in the "Date Sample Collected" column
  2. On the 21-day contact tracing printout (when you select "Print 21-day Report" on the Contacts-->Daily Follow-ups page, the vertical dates in the 21-day grid for each contact are still in DD/MM/YY order (day above month above year). They should have the month as the top number and day as the middle number.
